Organizational Scope

This policy applies to all full-time and part-time students in all programs in all settings.

Purpose

The purpose of this policy is to outline the processes surrounding student transcripts at the Michener Institute.

Policy

The Michener Institute for Applied Health Sciences (Michener) maintains official transcripts documenting the academic history of all students. Transcripts are available from the Office of the Registrar and Student Enrolment upon receipt of written request.

Transcripts are not issued to students who have outstanding tuition fees, residence fees, or other liabilities, owing to Michener; including fines owed to the Learning Resource Centre or other departments, or who have not returned Michener loaned equipment or resources.

Courses with formal evaluation where approved grades are issued by the Office of the Registrar and Student Enrolment are recorded on student’s transcript.

The Office of the Registrar and Student Enrolment cannot release high school transcripts and other confidential documents. Students must contact the institution that originally awarded the document should they wish to have a personal copy. Upon receipt of these documents for evaluation they become part of the permanent record at The Michener Institute.

Transcripts will not be issued to a third party without the written authorization of the student. A transcript is a confidential document. In order to ensure that records are kept secure, a student’s signature is required for verification purposes before copies of an academic record can be released. For that reason we cannot accept transcript requests via e-mail or by phone.  Due to saftey features of the transcript paper, transcripts cannot be sent via fax, or scanned and sent as pdf or any other type of electronic attachment.. All transcripts will be sent by regular mail or be available for pick-up from the Registration Office.

Official transcript requests must be made in writing.

For students who have completed both full-time program of study and select CE courses, all of the courses undertaken will appear on the transcript. However, non-evaluated workshops and seminars completed through the Continuing Educating department at the Michener Institute will not appear on the academic transcript. Students who attend and complete these activities will receive a Certificate of Attendance.

Procedure

To obtain an official transcript, students must complete a Transcript Request Form and submit it to the Office of the Registrar and Student Enrolment, together with a payment of $10.00 for each transcript. These forms are available in the Registrar’s Office or on the Michener website at http://my.michener.ca/forms/Transcriptreq.pdf

Additional fees are required to mail transcripts by Priority Post or Special Delivery:

  • $10.00 for each transcript sent to an address within Canada
  • $25.00 for each transcript sent to an address within the U.S.A.
  • $50.00 for each transcript sent to an international destination
