Scarborough Academic Family Health Team
The SAFHT delivers a suite of health programs to over 36,000 patients in Eastern Toronto. SAFHT’s team consists of 22 family physicians and their staff as well as SAFHT professionals: nurse practitioners, a physician assistant, registered nurses, a mental health team, dietitians, a psychiatrist, a chiropodist and a pharmacist. Over 60% of its clientele are high needs patients (they have complex and chronic diseases).As a result, SAFHT has created six Chronic Disease Management Programs: Diabetes Management with a Chiropody service, Hypertension Management, Mental Health Management, Coronary Artery Disease/Congestive Heart Failure, Focused Complex Vulnerable Services and Asthma/Chronic Obstructive Pulmonary Disease; and four Health Promotion Programs: Healthy You (for patients with low physical activity and bad eating habits), Heart Healthy (for patients with cardiovascular issues and bad eating habits); a Healthy Aging Program and Smoking Cessation. The formal enrolment in SAFHT programs is done by physician referral.The SAFHT is part of a network of Family Health Teams engaged in primary care community support and health care service delivery funded by of the Ontario Ministry of Health and Long-Term Care. This network includes Family Health Teams (FHT), Local Health Integration Networks (LHIN), Hospitals, and Health Links etc.
Position may alternate between two SAFHT offices
The Chiropodist provides full scope chiropody services to clients referred by the family physicians and allied health professionals within the family health team. The Chiropodist also participates in the Diabetes Management Program by providing comprehensive foot and wound care services to complex and high risk clients with diabetes.
The community chiropodist is a primary health care provider regulated under the provisions of the Bill 45 of the 1991 Regulated Health Professions Act, Province of Ontario. The chiropodist, subject to the terms, conditions and limitations of his/her certificate of registration may perform certain authorized acts.
Roles and Responsibilities:
This individual will play a supportive role within the SAFHT particularly in the assessment of the foot, treatment and prevention of diseases, disorders or dysfunctions of the foot by therapeutic, orthotic or palliative means.
Through education and awareness the chiropodist promotes foot health.
Work as an integral member of the interdisciplinary team, the chiropodist acts as a consultant, participating in holistic client-centered care provided by the Scarborough Academic Family Health Team.
Qualifications and Experience:
Completion of Bachelor of Science and Graduate - -
Advanced Diploma of Health Sciences (Chiropody).
Two to three years of clinical experience preferred.
Member in good standing with the College of Chiropodists of Ontario without any limitations.
Computer skills are required (Microsoft). Experience with EMR would be an asset.
Current Ontario Drivers’ Licence.
Experience in a family practice setting would be an asset.

Experience working with medically compromised clients (e.g. Diabetes, CVD, etc).

Knowledge of the Occupational Health and Safety Act and other relevant legislation required.
