Position SummaryThe HPE LLP Coordinator is responsible for the effective management of processes and resources as they relate to the care of patients in Hastings Prince Edward requiring referral and specialty care for diabetes management, vascular compromise and wound management. This role receives referrals from community partners providing care for patients with diabetes and other diseases which can result in lower limb compromise. Incumbent to this role is familiarity with the communities of practice salient to the prevention and management of lower limb wounds.This role requires an individual who is collaborative, patient focused and able to work with community and hospital partners to develop, implement and operationalize a standardized model of care that leads to reducing the incidence of lower limb amputations.

Duties
• Collaborates with Hastings Prince Edward Lower Limb Preservation Core Team to implement an integrated diabetes management and lower limb preservation care model
• Receive referrals from community clinics; determine using referral and patient interview information, how to optimize resources for patient and families and navigate care
• Work with a team to focus on early identification of patients at risk for vascular compromise, implement/support access to diabetes foot care and effective wound care
• Facilitate access to diabetes foot care and effective wound care for vascular compromised patients
• Collaborates with team to develop Fast track referral and triage process for patients requiring vascular surgery consultation including urgent referrals
• Engage with patients and families to implement collaborative care model
• Act as a program ambassador and educate patients, families, and staff on the program
• Support and educate hospital and community care providers regarding initiatives designed to maximize outcome for patients with vascular compromise
• Participate in and support Implementation of Patient Reported Outcome Measures and Patient Reported Experience measures and well as other data collection required to demonstrate the value of the program
• Identify and report any opportunities for program improvement

Requirements
Required
• Degree in a regulated health profession
• Current member of applicable regulatory College in good standing
• 1 year experience in last 5 years liaising with community partners or coordinating community home care
• Knowledge of community and government resources and associated legislation
• Exceptional assessment, negotiation, and problem-solving skills
• Exceptional interpersonal, communication, organization, and time management skills
• Ability to build and maintain relationships with community clinics, community partners, hospital clinics and other external stakeholders and optimize care delivery
• Ability to work in a fast-paced environment
• Ability to work independently and collaboratively with a team
• Effectively works within a team across multiple programs
• Ability to collect, synthesize and report on patient care data
• Ability to wear personal protective equipment effectively as required
• Demonstrated commitment to customer service and teamwork
• Supportive of a practice model that encourages accountability, communication, value for money, mutual respect, and cooperation
• Proficiency with computer and software is essential
• Demonstrated attention to detail with documentation, data collection and stakeholder reporting
• Reports to HPE LLP Project Operations Lead, Quinte Health
