Peterborough Regional Health Centre (PRHC) is a state-of-the-art regional hospital with a proud local history extending back more than a century. Funded by the Province of Ontario through the Central East Local Health Integration Network, PRHC opened a new, single facility in 2008, serving a population of 600,000 people in Peterborough and the surrounding region. With a capacity of 494 beds, PRHC has one of the busiest Emergency Departments in Ontario and offers an extensive range of services, including specialized programs in cardiac care, cancer care, and vascular surgery. The Health Centre is also the region’s largest employer, with more than 2,200 staff, 400 physicians with privileges, and 600 volunteers. The Diabetes Program Support Partner (PSP) coordinates and facilitates organizational efforts to improve the quality and safety of care related to diabetes. With strong diabetes clinical expertise and in collaboration with staff, physicians and patients, the PSP will support the development, delivery and evaluation of an in-patient diabetes improvement strategy. Key areas of focus will include the development of diabetes-related education and practice materials, including policies, order sets, care pathways, and directives. In addition, the PSP will help develop approaches to enhance diabetes care and self-management. Utilizing Diabetes Canada Best Practice guidelines, the PSP will lead the implementation of education and practice improvements in an effort to enhance safety and reduce length of stay. As an expert, the PSP will play a key role in clinical coaching/mentoring and leadership role-modelling. Approximately 80% of the Diabetes PSP role will be dedicated to content development and knowledge transfer to care providers throughout the healthcare facility.
•Required◦Current registration in good standing from the College of Nurses of Ontario
◦Baccalaureate of Nursing or equivalent
◦Canadian Diabetes Educators designation (CDE) current
◦Three years of recent clinical nursing experience in outpatient diabetes clinic and/or consultant role in diabetes education for staff and patients
◦BCLS provider certification
•Preferred◦Completion of a Master’s Degree in Nursing Education or equivalent is preferred
◦Completion of Adult Education Courses
◦Previous teaching experience with nurses in both classroom and clinical setting preferred
◦Completion of a Leadership Program
◦Choices and changes
◦Membership in related professional associations RNAO, Diabetes Canada
•Demonstrates excellence in clinical nursing practice, critical thinking and problem solving skills
•Demonstrates ability to adapt to new/emerging technologies/best practices at the bedside (eg MEDITECH, ADU, IBT, e-documentation; Best Practices, Safer Health Care Now Initiatives)
•Proficient ability to assess staff learning needs and to set learning goals and objectives
•Excellent leadership skills, in particular role modelling, coaching and mentoring skills
•Proficient Computer skills (eg. E-mail, Word, Excel, Internet, Power Point)
•Superior verbal and written communication skills.
•Strong interpersonal skills and ability to work effectively with staff, physicians, administrators and the community.
•Good knowledge and skill in curriculum development/delivery
•Good knowledge and skill in adult teaching/learning theory
•Demonstrated ability to work independently and in an interprofessional environment
•Demonstrated ability to be flexible and open to others’ ideas
•Demonstrated ability to network with internal and external networks/partners
•Demonstrated ability to maintain a good attendance record and meet the performance standards of the Health Centre
