Duties
Position Summary:
Responsible for development of nutrition care plans and counselling for patients with diabetes.
Work under medical directives to work at full scope of practice
Ability to teach and assess all aspects of diabetes management, including insulin initiation, capillary blood glucose and senor devices, medication management, foot assessments and other diabetes standards of care.
Use a variety of teaching methods, incorporating the principles of adult education and a variety of client engagement or empowerment modalities to facilitate behaviour change, i.e. MI, stages of change
Broad understanding of nutrition resources available in the community to support clients with complex needs
Regular rotations in the inpatient units at the Credit Valley Hospital to support bedside teaching for diabetes and safe discharge
Functions as part of an inter-disciplinary team and acts as a resource for other health care providers
Facilitate group education programs at the DMC
Develops nutrition education resources
Participates in program planning and evaluation
Requirements
Key Qualifications:
THP is committed to internal talent, and therefore priority will be given to internal applicants.
Degree in Human Nutrition and Dietetics combined with an accredited Dietetic Internship program
Member in good standing with the College of Dietitians of Ontario
Current membership in Dietitians of Canada and Diabetes Educator Sector of Diabetes Canada preferred
Certified Diabetes Educator preferred
Minimum of 3 years current experience working with clients with diabetes in a health care setting.
Good working knowledge of diabetes medications including insulin required
Experience using medical directives as asset
Good working knowledge of diabetes and other comorbidities through the lifecycle and adult teaching principles
Individual counselling and group counselling experience preferred
Demonstrated leadership and communication skills
Positive work and attendance record essential
Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively within a team environment
A second language an asset
Hours
Monday - Friday | 8-4pm
Salary
AH10.2 ($34.90 - $44.46)
