Humber River Family Health TeamThe Humber River Family Health Team (HRFHT) provides primary health care, including chronic disease management and health promotion and disease prevention, to a rostered population of patients in the North West Sub Region of the Central LHIN region of the GTA. The FHT utilizes an interdisciplinary team of healthcare professionals who work in a collaborative model of care.

The Chiropodist is integral in the care of the patients of the HRFHT. The Chiropodist will assist the FHT in achieving its vision and mission. The Chiropodist will support patient care and the delivery of programs, including chronic disease and geriatric, are, by focusing on the delivery of effective foot care.
This is a full-time position, based currently out of our Weston and Lawrence location.
Roles and Responsibilities:
Accept all patients referred by the FHT interdisciplinary health providers, in accordance with the FHT’s established referral protocol, to provide appropriate foot care.
As a primary care provider, prepare therapeutic and management plans with patients in consultation with the team.
Implement, monitor and follow up foot care treatment and plans in accordance with the Chiropodist scope of practice and in collaboration with FHT interdisciplinary care providers.
Develop, update, implement and maintain education resources for patients and for providers.
Will be required to work at more than one FHT site.
Assist in the planning, implementation and evaluation of chronic disease and health promotion and illness prevention programs and services.
Collection of data as required for statistical/reporting purposes.
Identify gaps and needed modifications in services.
Facilitate referrals to other services as well as communication and networking between the primary care practice and hospital or home care.
Represent the mission, values and philosophy of the FHT in the community, including networks, meetings, or presentations, as requested.
Assist in the training and supervision of students and/or volunteers as requested by the Executive Director and/or Lead Physician.
Qualifications and Competencies:
B.Sc. and licensing/registration by the College of Chiropodist of Ontario. Minimum one year of demonstrated clinical experience in a community setting is required.
Additional training or knowledge in the areas of chronic disease management and geriatrics is considered an asset.
Demonstrated clinical skills in assessment, program planning, implementation and evaluation.
Work effectively, both independently and as part of an inter-disciplinary team.
Proven flexibility and ability to adapt to change.
Computer skills. Experience with EMR an asset.
Ability to work flexible hours (evening and weekend work is required)
