About the Organization: The Humber River Family Health Team (HRFHT) is a non-profit organization providing interdisciplinary, patient-centered primary care with the goal of optimizing individual and family health within the community. We provide primary health care, including chronic disease management and health promotion and disease prevention, to a rostered population of patients in the northwest sub region of the greater Toronto area. The HRFHT utilizes an interdisciplinary team of health care professionals who work in a collaborative model of care. Roles and Responsibilities:
1. Assessment of foot disorders or dysfunctions and treatment of same therapeutic, orthotic or palliative means;
2. Differentiating between disorders of the foot arising from systemic or general disease and those arising from local, structural and/or functional disorders;
3. Recognizing patients having conditions beyond their scope of practice and refers to other appropriate health care professionals (e.g. primary care provider);
4. Designing and implementing a management plan based no presenting conditions and the patient’s individual needs. The Chiropodist will reinforce and encourage patient responsibility and participation in the management plan;
5. Communicating foot health education in a variety of settings to patients, Interprofessional health care professionals (IHPs) or other concerned individuals;
6. Ensuring appropriate antiseptic techniques, safe procedures and isolation practices are maintained in the clinical and/or surgical setting; and providing a safe environment for patients, practitioners and others in the clinic.


Qualifications and Competencies:
1. B.Sc. and licensing/registration by the College of Chiropodist of Ontario.
2. Additional training or knowledge in the areas of chronic disease management and geriatrics is considered an asset.
3. Demonstrated clinical skills in assessment, program planning, implementation, and evaluation.
4. Work effectively, both independently and as part of an inter-disciplinary team.
5. Proven flexibility and ability to adapt to change.
6. Computer skills. Experience with EMR an asset.
7. Ability to work 1 evening per week and 1 Saturday per month.

Start Date



9-5pm, 1 evening shift a week and 1 Saturday shift per month


$70,000-74,000, plus access to benefits and RRSP plan
