AC-SSN-POL-001
Student Incident Reporting Policy
Approval DateJune 28, 2018
Effective DateSeptember 4, 2018

Organizational Scope

The Policy applies to all full-time and part-time students enrolled in programs* offered in The Michener Institute of Education at UHN (“Michener”).

*Students in joint programs must refer to their program handbook.

Purpose

This policy is a guideline for reporting accidents, near misses or incidents that occur involving students while on Michener property or while at a clinical site.

Policy

Students who incur an injury or other medically related incident while at The Michener Institute must seek medical assistance immediately from a local walk-in clinic or the nearest emergency department. If life-threatening, call 9-1-1.

Students who, while at a Clinical Site, incur an injury or other medically related incident, including an incident that may place them at risk of acquiring a communicable disease, must seek medical attention after the patient’s safety is ensured.

The reporting of incidents assists in the prevention of any further accident or injury as the cause may be preventable in the future.
