Purpose
Reporting of a student Incident or near miss.
Click here to report any sexual assault, sexual violence or sexual harassment incident.
Procedure
Incident incurred at Michener:
- The student will report any single incident or near miss to the supervising faculty/staff member. The student will seek medical attention as required.
- Within 24 hours of the incident occurring, the supervising faculty/staff member in collaboration with the student, will file an incident report with Michener Health Services.
- Within 72 hours of the incident occurring, Health Services will follow-up with the student and undertake appropriate action including reporting to the WSIB if required. A copy of the Incident report will be forwarded to the Academic Chair.
Incident incurred while at a clinical placement site*:
- The student will report any single incident or near miss to the supervisor/clinical coordinator
- Within 24 hours of the incident, the student in collaboration with the clinical coordinator will complete a Student Incident Report Form. The incident report must be forwarded to the student’s Clinical Liaison Officer at Michener and Michener’s Health Services either via email to healthservices@michener.ca or alternatively faxed to HealthServices at (416) 596-7214
- Within 72 hours of the incident occurring, Health Services will follow-up with the student including reporting to the WSIB if:
- a student is seen by a medical professional
- a student is unable to attend clinical placement due to the incident
- work or academic accommodation is required due to the incident
*Clinical Placement Site policies and procedures may supersede Michener’s
Consequence
Incident reports are mandatory and failure to submit will be addressed by the students’ Academic Chair.
Failure to submit a WSIB recordable incident, will result in penalties per WSIB Regulations.
