Supporting Documents

Offers of admission will be made to students based on a combination of information received from submitted documents. Any information received late, or not at all, will affect our ability to evaluate your academic and work-related credentials and will severely impact your offer of admission from Michener. It is expected that offers of admission will be issued in April of 2022.

  • Michener requires full disclosure of an applicant’s postsecondary academic history. Please note that Michener does not review documents prior to applicants applying to Michener programs.
  • Original or notarized copies are required for all documents. Photocopies or email attachments are not considered official documents and will not be accepted.
  • Internationally educated applicants must have their credentials evaluated (course-by-course evaluation) by World Evaluation Services (WES) or International Credential Assessment Service of Canada (ICAS) and then submitted with an application to Michener.

Graduate Programs

Cardiovascular Perfusion

Applications for the Master of Science in Cardiovascular Perfusion will open via Michener Self-Service in November 2021. Applicants must submit the following documents:

Option A:

  • Postsecondary transcripts (Official Notarized copies will also be accepted.)
  • Additional proof of prerequisite completion if completed outside postsecondary transcripts
  • Complete Cardiovascular Perfusion Applicant Experience Checklist
  • Supporting documentation, if necessary, for the Applicant Experience Checklist
  • Applicants for whom English is a second language must provide proof of an English language assessment.
  • Letter of Intent

Option B:

  • Proof of registration documents with CRTO or CNO
  • Complete Cardiovascular Perfusion Applicant Experience Checklist
  • Supporting documentation for the Applicant Experience Checklist
  • All documents relevant to training and/or experience (resume or CV)
  • A letter from an employer stating at least one year of critical care experience within the last five years
  • Applicants for whom English is a second language must provide proof of an English language assessment.
  • Letter of Intent

All applicants are asked to fill out an Applicant Experience Checklist and upload the requested supporting documents along with the form on Self-Service by February 8, 2022.

Full-Time Programs

Chiropody

Applicants to the Chiropody program must submit the following supporting documents by February 8, 2022.

  • Postsecondary transcripts (Official Notarized copies will also be accepted.)
  • Additional proof of prerequisite completion if completed outside postsecondary transcripts.
  • Applicants for whom English is a second language must provide proof of an English language assessment.

    Diagnostic Cytology

    Applicants to the Diagnostic Cytology program must submit the following supporting documents by February 8, 2022.

    • Proof of completion of college/university level Basic Statistics course if completed outside postsecondary transcripts.

    Genetics Technology

    • Additional proof of prerequisite completion within last 10 years if completed outside degree/diploma
    • Review the Genetics Presentation and Complete Genetics Technology Applicant Experience Checklist

    • Applicants for whom English is a second language must provide proof of an English language assessment.

    Option B:

    • Proof of Canadian certified Medical Laboratory Technologist (MLT) diploma with a minimum CGPA of 75% via official transcript
    • Proof of registration documents
    • Additional proof of prerequisite completion within last 10 years if completed outside degree/diploma

    Radiation Therapy (University of Toronto Joint Program)

    Applicants to the Radiation Therapy program must submit the following documents via OUAC:

    Radiological Technology (University of Toronto Joint Program)

    Applicants to the Radiological Technology program must submit the following documents via OUAC:

    Respiratory Therapy

    Applicants to the Respiratory Therapy program must submit the following supporting documents by February 8, 2022.

    • Applicants for whom English is a second language must provide proof of an English language assessment.

      Ultrasound

      Applicants to the Ultrasound program must submit the following supporting documents by February 8, 2022.

      Option A:

      • Additional proof of prerequisite completion if completed outside postsecondary transcripts
      • Applicants for whom English is a second language must provide proof of an English language assessment.

      Option B:

      • Proof of registration in regulated health profession with minimum 3 year degree/diploma with a minimum CGPA of 75% via official transcript
      • Proof of registration documents.

      Part-Time Programs

      Anesthesia Assistant Graduate Certificate

      Applicants to the Anesthesia Assistant Program must submit the following documents by June 7, 2022:

      • Proof of registration with CRTO or CNO
      • Current resume describing background and experience
      • A letter from your Chief of Anesthesia supporting return to home base site for clinical education. Please note that all students are required to complete a minimum 2 week elective rotation external to the assigned base site

      How to submit:

      Magnetic Resonance Imaging (MRI) Graduate Certificate

      Option A:

      Applicants to the Magnetic Resonance Imaging must submit the following documents by June 7, 2022

      • Postsecondary transcripts (Official Notarized copies will also be accepted.)
      • Proof of Certification/licensure in one of the following four disciplines:
        • Nuclear Medicine (RTNM) – certification from CAMRT or equivalent
        • Radiological Technology (RTR) – certification from CAMRT or equivalent
        • Radiation Therapy (RTT) – certification from CAMRT or equivalent
        • Ultrasound (RDMS) – Please note that RDCS certification does not meet the admission requirements of this program

      Option B:

      Internationally Educated Medical Radiation Technologists (IEMRTs) and Sonographers:

      • Postsecondary transcripts (Official Notarized copies will also be accepted.)
      • Internationally educated medical radiation technologists (IEMRTs) must provide a WES (course-by-course) or ICAS (comprehensive) evaluation
      • Applicants for whom English is a second language must provide proof of an English Language Assessment
      • Internationally educated and trained applicants must meet the corresponding Canadian certification body requirements and should consult with:
        • Canadian Association of Medical Radiation Technologists (CAMRT) for the Internationally Educated Medical Radiation Technologist (IEMRT) requirements, or;
        • Sonography Canada’s External Candidate requirements.

      How to submit:

      • Official transcripts, as well as credential evaluations from ICAS or WES must be submitted directly to OCAS (physical/electronic) by the educational institution or evaluation service.
      • English language assessments must be sent directly to ask-us@ontariocolleges.ca by the assessment provider.
      • All other supporting documents with the exception of official transcripts, and credential evaluations must be submitted by the applicant to ask-us@ontariocolleges.ca

      Advanced Foot and Wound Care

      Applicants to the Advanced Foot and Wound Care program must submit the following documents via TBD:

      • Proof of graduation from a recognized Chiropody/Podiatry program
      • Proof of membership in good standing with the College of Chiropodists of Ontario (COCOO)
      • Resume and Chiropody wound care portfolio
