Required supporting documents vary program to program. Before applying, please review the information below to determine which documents you need to submit to support your application.

Important notes before you prepare your application:

  • Michener requires full disclosure of an applicant’s postsecondary academic history. Please note that Michener does not review documents prior to applicants applying to Michener programs.
  • Original copies are required for all education documents. Photocopies or email attachments are not considered official documents and will not be accepted. Email transcripts are only considered official when sent directly by the institution.

Internationally educated applicants must have their credentials evaluated (course-by-course evaluation) by World Evaluation Services (WES) or International Credential Assessment Service of Canada (ICAS) .

Graduate Programs

4) Letter of Intent

  • All applicants are required to submit a letter of intent to support their application.
  • The letter of intent will assist the Admissions Committee to better understand an applicant’s academic and professional trajectory and future career goals and aspirations as well as to assess the applicant’s knowledge within the Cardiovascular Perfusion field.
  • The letter must not be more than two (2) pages in length.

5) Letter of Recommendation (Academic or Professional)

  • All applicants are required to submit one (1) letter of recommendation.
  • The letter can be from either an Academic or Professional Referee and should indicate why you would be an excellent fit for the program. They should specifically draw on your experience and how that has prepared you for success in the program and in a career as a Cardiovascular Perfusionist.

    Part-Time Programs

    Applicants must submit the following documents:

    1) Educational Requirement

    • Official transcript showing degree/diploma conferral from a recognized Chiropody/Podiatry program

    2) Licensure Requirement

    • Proof of membership in good standing with the College of Chiropodists of Ontario (COCOO)
      Accepted forms of proof: recent receipt, screenshot of licensure portal, letter from licensing body indicating licensure, etc.

    3) Experience Requirement

    • Resume indicating a recommended minimum of two (2) years of clinical practice
    • Chiropody wound care portfolio

    Where to submit documents

    • Physical or electronic official transcripts, as well as credential evaluations from ICAS or WES must be sent by the educational institution or evaluation service directly to Michener or admissions@michener.ca
    • All supporting documents with the exception of transcripts must be submitted to admissions@michener.ca

    Applicants must submit the following documents by June 7, 2024:

    1) Licensure Requirement

    • Proof of registration with CRTO or CNO
      Accepted forms of proof: recent receipt, a screenshot of licensure portal, a letter from licensing body indicating licensure, etc.

    2) Critical Care Experience Requirement

    • Current resume describing background and experience
      • Resume must indicate 2 years or full-time equivalent (>4000 hours)* of critical care experience within the past 4 years, which must include experience with therapeutic interventions related to airway management, mechanical ventilation, arterial or intravenous access, reading ECGs, providing pharmacological therapies, and managing hemodynamic status, ideally in the ICU, ER, NICU, or PICU environments.

    *Accrual of critical care experience begins after the applicant is licensed. Experience does not include any in-class clinical hours.

    3) Support Letter Requirement

    • A letter from your Chief of Anesthesia supporting return to home base site for clinical education. Please note that all students are required to complete a minimum two-week elective rotation external to the assigned base site

    Applicants must submit the following documents by June 7th, 2024:

    1) Licensure Requirement

    Proof of Certification/licensure in one of the following four disciplines:

    • Nuclear Medicine (RTNM) – certification from CAMRT or equivalent
    • Radiological Technology (RTR) – certification from CAMRT or equivalent
    • Radiation Therapy (RTT) – certification from CAMRT or equivalent
    • Ultrasound (RDMS) – Please note that RDCS certification does not meet the admission requirements of this program
      Accepted forms of proof: recent receipt, a screenshot of licensure portal, a letter from licensing body indicating licensure, etc.

    Please Note: Internationally educated and trained applicants must meet the corresponding Canadian certification body requirements and should consult with:

    • Canadian Association of Medical Radiation Technologists (CAMRT) for the Internationally Educated Medical Radiation Technologist (IEMRT) requirements, or;
    •  Sonography Canada’s External Candidate requirements.
