To submit official requests to the Registrar’s Office please access the forms below.

Please be sure to review all relevant policies and procedures prior to completing your request.

Form Description
Application for Readmission Form Student applies for readmission.
Bursary Application Student applies for a bursary.
Exemption/Transfer Request Form Student applies for transfer credit by course.
Official Withdrawal Form Student withdraws from program.
OSAP Tuition Deferral Form Student receiving OSAP requests that their tuition due date be deferred until disbursement of funds.
Request for Confirmation of Enrolment Student requests an official letter drafted by the Registrar’s Office attesting to Michener student status.
Request for Replacement Diploma Student requests an official diploma to replace a lost/missing one.
Request for Transcript Student requests an official or unofficial transcript to be mailed by the Registrar’s Office.
Scholarship Application Form Student applies for a scholarship.
